Our forms allow you to report things, make applications, book online or get in touch.

Your details

In some instances, it is important that we have your details so that we can action your request and contact you.

When the form asks you to enter your address, press the Get address button and it will take you to a search page where you can choose your address.

Medway residents

If you are a Medway resident, you will be able to search for your address either by the property name or number, the street or the postcode.

If you know your postcode, enter this first, and then press the Find address button. If you are unsure of your postcode, try searching using the name or number of your property or street name.

If you are unable to find your address using the search functions, press the Enter address button and this will allow you to enter your address manually.

Live outside Medway

If you are not a resident of Medway, tick the Is your address outside of the Medway area? box so that you can enter your address manually.

If you have entered an address incorrectly, press the Clear button. This will remove the address you have selected and allow you to search again.

Entering the details of a location

If you want to contact us about a location within Medway because it is damaged and in need of repair or some action by us, or because you wish to make changes to the location because it is your property, it is important that you provide us with the accurate location address.

You can search for the location address in the same way as you would your own address. If you are not sure of the exact address, please enter the nearest address or street.

Uploading documents

If you have to provide us with copies of documents to support your application, you can upload these documents to the eform.

When an eform asks for documents to be uploaded, a Browse button will appear.

By clicking on this, you will be able to navigate to the file on your computer. Once you have found the file, press ‘upload’ and the file will be added to the eform.

Submitting and printing a form

Once you have completed the form, you press the ‘submit’ button. If you do not press Submit and close the eform, then the information you have entered will not be sent to us.

The Print button at the end of each form allows you to print a copy of the completed eform. You will be able to print the form both before and after submitting the eforms.
